Emma Dartis

Emma Dartis has a Bachelor’s of Science degree in Psychology and a Master’s of Social Work degree both from Indiana University Purdue University in Indianapolis. She is a Licensed Clinical Social Worker and has over thirty years of experience providing case management, therapy, and care coordination services in the areas of children and family, foster care, home based, group home, work release, and managed care environments. Emma is currently the Transition Coordination Manager at CareSource, which includes the re-entry and transition programs. The transition program at CareSource consists of a multi-disciplinary team of nurses, social workers, and Community Health Workers who assist individuals who have had either a medical or behavioral health hospitalization to transition home successfully after a discharge. The re-entry program at CareSource consists of Criminal Justice Liaisons and a Community Health Worker that provides a continuum of care for justice involved individuals from pre-release up to 90 days post release after an incarceration.
